Thyroid certification requires a blood test. A copy of your pet’s registration should be emailed to us no later than one day prior to your dog’s appointment. Emailing us a copy of the registration (AKC, CKC or other) helps ensure accuracy of the submission, avoiding delays in receiving your certification. Information required:
- FULL registered name, registration number (AKC, CKC or other) registration numbers for Sire and Dam
- Date of birth
- Microchip or tattoo number
- Owner and Co-owner information – name, address, phone number, email.
OWNER OR CO-OWNER LISTED WILL BE REQUIRED TO SIGN THE OFA FORM AT THE TIME OF ADMISSION.
There will be a separate charge for submission of the form, payable to OFA. Payment by credit card is preferred and will be required at the time of signature. Checks are acceptable, but processing can take longer.
